You will be responsible for all co-pays, deductibles and/or non-covered expenses at the conclusion of today's visit. If you have any questions about the cost of services provided to you today, please ask your provider for an estimate of the the amount you will be expected to pay.
If you are unable to meet your financial responsibility for the services you are to receive today, we will be happy to reschedule your appointment to a later date.
Additionally, effective July 1st, all surgery patients will be required to meet all deductables for their portion of the surgeon's fee PRIOR to surgery. You can expect your surgery to be cancelled or rescheduled if your portion of your surgery fee is not paid prior to surgery.
We accept cash, personal checks and major credit/debit cards.
